Community Care

We understand that not all patients wish to come into the Hospice to be cared for hence the valuable service that our community teams can offer to patients who choose to remain at home, in their own surroundings.

"A Hospice without Walls"

Our community services consist of two teams:
Clinical Nurse Specialists and Hospice at Home

Their main aim is to help patients who choose to remain at home during their illness achieve this.

The service provided by our community teams maintains the same ethos of care delivered by Willen Hospice: 

• Maintain dignity
• Assist in controlling symptoms
• Enable patients to make decisions about their care 

The community teams are based at Willen Hospice.  They work in partnership with General Practitioners, District Nurses and other health and social services,although the patient's own GP and District Nurse remain responsible for the overall care at all times.

Their outreach covers a large geographical area.
