Hospice at Home Service

The team consists of a combination of 10 experienced registered nurses and health care assistants who have cared for patients with life limiting illness.

The Hospice at Home team offers practical support and nursing skills to patients at home. The team are able to assess and assist other health care professionals to control patients symptoms help families. In addition, some member of the team can offer complementary massage.

The team work in conjunction with other professionals – District Nurses; Twilight Nursing Team and GPs, although the District Nurse remains responsible for overall care at all times.

The service is unique as patients can contact the team for advice and support from 07.30 to 20.00 seven days a week.
