The Social Care Department is able to offer support to you, your family and your carers.
This support may be regarding personal or family issues, practical or emotional problems, or discharge planning for patients in the Hospice.
We work closely with other members of the Hospice multi-disciplinary team and with outside agencies such as Social Services, Local Health care Professionals, Advisory Services etc. If we are unable to help we have access to a wide range of information about resources and are usually able to find someone who can.
are all part of the Social Care Department. With the recent addition of a second Social Worker to the Department, we are hopeful that we will be able to extend the range of support we offer, particularly to carers.
The Department is also involved in facilitating parts of the Hospice training programme. We offer adult and child protection awareness training to Hospice staff and volunteers. We also offer training about psychosocial care and working with children and families to other professionals.

If a member of the team is not available please leave a message and your call will be returned as soon as possible. The Department is open during normal office hours.
