Our Willen at Home nurses continue to care for our patients in the community.
We are committed to supporting our patients during Covid-19. While this is not always possible in person at the moment, support is provided over the phone and by video. The role this team plays is critical in making sure that as a patient, you can live with your illness at home, in your own surroundings. Our Willen at Home team can offer you a valuable service if you choose to remain at home, and we recognise that not everyone will want to come into the Hospice for their care. So our main is to help you to remain at home during your illness if that is your wish. We will help you to maintain your dignity, assist you in controlling your symptoms and help you to make decisions about your care.
We work in partnership with General Practitioners, District Nurses and other health and social services, although as a patient, your own GP and District Nurse remain responsible for your overall care at all times. Our specialist service is based on assessing and meeting your needs, and those of your family.
Who are we?
We are a multi-professional palliative care team made up of of Clinical Nurse Specialists, Registered Nurses, Healthcare Assistants and Specialist Doctors, based at Willen Hospice. We visit you in your own home or see you in the outpatient clinic at Willen Hospice.
What do we do?
We work with you and your family to identify your health and care needs and do our utmost to ensure these are met. We'll support you to plan for the future so that as far as possible you get the care you want in the place of your choice.
Your wishes will be recorded in an Advance Care Plan so that everyone involved in your care knows what is important to you.
We are specialists in managing complex symptoms and understand that your psychological and spiritual needs are just as important as physical ones.
We work closely with our In-Patient Unit and where needed we'll refer you to other professionals in the Willen team such as the Physiotherapist, Lymphoedema Nurses, Spiritual team or Therapeutic and Wellbeing teams. If your symptoms are not settling with management at home, it may be possible to admit you to the Hospice for a few days so that you can be monitored more closely.
Who can refer patients to Willen at Home?
We accept referrals from healthcare professionals, including GPs, Hospital Consultants, Hospital Advanced Nurse Practitioners and District Nurses.
