Willen at Home

Caring for our patients in the community

Our Willen at Home team (WAH) includes Clinical Nurse Specialists, Registered Nurses, Doctors and Health Care Assistants who all have specialist training and experience in caring for people with a life limiting illness. We work alongside GPs and District Nurses to give advice on managing any difficult symptoms, with a focus on quality of life.

We see people at different stages of their illness, including those nearing the end of life, and offer support to families and carers. Many people with life limiting illnesses do not need our involvement as GP and District Nurses are able to provide the care and support needed and remain responsible for overall care.

Referrals

Referrals to our service can be made by any healthcare professional when a person’s needs are more difficult to manage and require specialist involvement. Only people under a Milton Keynes GP can be referred to WAH.

We support people to record their wishes in an Advance Care Plan so that everyone involved in their care knows what is important to them. We also support people to plan for the future so that as far as possible they can be cared for in the place of their choice.

Onward referrals may be made to the Willen Therapeutics & Wellbeing team to support with psychological, spiritual and social needs, or to other services as needed.

If symptoms are not settling with management at home, it may be possible to admit to the Hospice for a few days for closer monitoring.
