Living Well Programme

What is the Living Well Programme?

The Living Well programme is a collection of different sessions and groups that are designed to provide you with the support and skills you need to manage your condition and maintain a good quality of life. It is tailored to meet your individual needs and goals.

What does it involve?

The programme begins with a short assessment with an Allied Health Professional and/or Nurse. At the end of the assessment, they will talk about your priorities and suggest sessions or groups that could be of benefit or refer you to an alternative service.

Wellbeing means different things to different people; so our aim is to offer a programme of varied support and activities within which we hope everyone will find something right for them.

Living Well Education

Living Well Education is a 6 week rolling course; each week focusing on a different symptom or issue commonly reported by people with life limiting conditions. The sessions are group based and will focus on education and self-management techniques that could help reduce the impact of these symptoms or issues on your day to day life.

Each week will include a face to face education session and either gentle exercise or relaxation, as well as a chance for social interaction and peer support. The opportunity to join virtually will also be available depending on the current situation and guidelines.

Topics include:

·         Managing Feelings & Emotions

·         Managing Change & Uncertainty

·         Fatigue

·         Breathlessness

·         Sleep

·         Eating & Nutrition

It is a good opportunity for you to meet members of the Willen Hospice Therapeutic & Wellbeing multi-disciplinary team and find out about what other services are available.

Virtual sessions will start in January and run every Tuesday at 12:30 or 13:00. This service is for anyone already under the care of Willen Hospice. Please contact a member of your Willen Team to be referred into the programme.
