Living Well Programme

What is the Living Well Programme?

The Living Well programme is designed to help support you and your family to live life the way you want to. Our specialist Therapeutic and Wellbeing team put you at the centre of your care, and work alongside you to offer services that are tailored to your needs.

Holistic Assessment

After referral to the service, you will be invited to come and see us for a holistic assessment. It is here that we talk to you about what matters most to you, any difficulties you are facing, and come up with some goals and areas you would like to focus on. From here we can suggest and recommend particular groups and sessions which will help you work towards your own individual goals.

Education Sessions

Each week will include a face to face education session based on commonly reported symptoms and/or issues. This will be delivered by a member of the Therapeutic and Wellbeing team. There is opportunity for meeting members of the Willen Hospice multi-disciplinary team, as well as sharing ideas and experiences with others, if you wish.

Topics include:

·         Managing Feelings & Emotions

·         Managing Change & Uncertainty

·         Fatigue

·         Breathlessness

·         Sleep

·         Eating & Nutrition

Living Well Activity Groups

Engaging in activities that are meaningful brings many benefits- from physical exercise, social support, developing skills, and promoting psychological wellbeing. Activities may be something you enjoy already, or may be something new that you haven’t tried before. Activity groups will have an individual topic and may include:

·         Horticulture / Gardening

·         Sewing / Craft

·         Seated Exercise

·         Standing Exercise

·         Balance and Falls

·         Yoga

·         Relaxation

Therapy Review

After approximately six weeks, you will meet again with a member of the team to talk about how you have been getting on. We will review your goals and talk about next steps. We may recommend further sessions, refer you to another service, or discharge you, depending on your own individual circumstances.
